Make sure you're banning all nick/IP combinations. If someone joins on the nick "PlayerA" with the IP of "x.x.x.x" and you ban them, then only "PlayerA" and "x.x.x.x" are banned.

If you do !nick x.x.x.x and !search_ip x.x.x.x and it comes back with "PlayerB, PlayerC, PlayerD" and those accounts have other IPs then they'll still be able to play.

Serial bans are apparently a thing of the past, and BR has some critical flaws in the way it bans. So you'll have to do a little detective work with each and every ban.

I don't have the time to investigate this so hopefully a co-admin does.
